چکیده

Vocal fold scarring can arise from disease or postsurgical wound healing and is one of the predominant causes of voice disorders. Focused ultrafast laser pulses have been previously demonstrated to create tightly confined sub-surface ablation in a variety of tissue, including vocal folds. Here, we demonstrate how we can take advantage of this unique ability of ultrafast laser ablation to create sub-surface vocal fold microsurgeries with a goal for eventually creating a plane in tough sub-epithelial scar tissue into which biomaterials can be injected. Specifically, we create sub-epithelial ablations in vocal fold tissue in under 1 minute using sub-μJ pulses from a compact, commercially available amplified ultrafast laser system from Raydiance Inc., operating at a 500kHz repetition rate. The use of relatively high repetition rates, with a small number of overlapping pulses, is critical to achieving ablation in reasonable amounts of time while still avoiding significant heat deposition. Additionally, we use multiphoton fluorescence of the ablation region and SHG imaging of collagen fibers to obtain visual feedback of tissue structure and confirm successful ablation. Lastly, we demonstrate microsurgery using amplified ultrafast laser pulses delivered through over 1 meter of air-core photonic crystal fiber to a laser scanning microsurgery probe, illustrating the feasibility of developing an ultrafast laser surgical laryngoscope. We aim to further develop this clinical tool through demonstration of laryngeal microsurgery using a compact laser system in conjunction with a larynx-specific fiber-based surgery probe. Towards endoscopic ultrafast laser microsurgery of vocal folds.

Vocal fold scarring is a predominant cause of voice disorders yet lacks a reliable treatment method. The injection of soft biomaterials to improve mechanical compliance of the vocal folds has emerged as a promising treatment. Here, we study the use of precise femtosecond laser microsurgery to ablate subsurface voids, with a goal of eventually creating a plane in dense subepithelial scar tissue ...

Parameters affecting ultrafast laser microsurgery of subepithelial voids for scar treatment in vocal folds.

Toward developing a new method for restoring tissue viscoelasticity in scarred vocal folds, we previously proposed a method to localize biomaterials within subepithelial voids ablated using ultrafast laser pulses. The clinical implementation of this method necessitates the quantification of the laser parameters for ablating scarred tissue. Here, we present a comprehensive study of these paramet...

Endoscopic CO2-Laser Surgery for Vocal Cord Cancer

A retrospective study is reported on endoscopic CO(2)-laser microsurgery in 69 patients with histologically verified early vocal cord cancer. A flexible nasopharyngolaryngoscope (STORZ Co) was used for preoperative assessment and occasionally for postoperative follow-up.Six years of experience with this technique have led to endoscopic cordectomy, previously not accepted as a therapeutic method...

The Study of Vocal Function in Patients With Early Laryngeal Carcinoma After Transoral Laser Microsurgery

Objective Today transoral laser microsurgery is considered as one of the first options to control early laryngeal cancer, and voice disorder is one of the inevitable complications of this therapeutic component. This study aimed to compare the vocal function in patients with early-stage laryngeal cancer following laser surgery with healthy individuals with normal voice quality using acoustic ana...

Kinematic analysis of a Meso-Scale Parallel robot for laser Phonomicrosurgery

The paper presents the kinematic model of a new meso-scale (∼1 cm) parallel kinematic machine intended for laser phono-microsurgery of the vocal folds. The proposed mechanism uses the displacement generated by piezoelectric cantilever actuators manufactured via a Smart Composite Microstructure technique. The architecture, the geometry, and the position kinematics of the device, modeled as a spa...
